I think she does this because it is a form of self harm kind of like how some people cut them selves. She was just trying to get over that her grandchildren had left and she decided to beat her head with a rock.
I think that the grandmother does this because she is trying to "beat" out the memories of her grandchildren being taken from her. She feels guilty and afraid, so beating her head with a rock is how she copes.
I think she is literally beating herself up for them being taken away. She thinks she could've done something about it. She was also a little hard on herself.
